Application for the Supply and Installation of a Memorial

This is an application for the supply and installation by Toowoomba Regional Council of a memorial at an interment site (a memorial includes a plaque).

This application is to be completed by a person who has a legal right to authorise the supply and installation of a memorial at the interment site. Interment site means a site in which a deceased person/s or deceased person's cremated ashes is buried/placed such as a grave, vault, crypt, mausoleum, niche, garden, etc.
